PLEASE NOTE: THIS CONFIRMATION IS FOR THE MEETING REGISTRATION ONLY, AND IS NOT A CONFIRMATION OF HOTEL RESERVATION. Hotel reservations at the CMSA room rate of $199.00 per night are not being placed through CMSA this year; instead they should be made directly with JW Marriott. Event Registration Refunds: To be eligible for any refund, Conference cancellations must be received on or before December 14, 2009 and are subject to a $100 cancellation fee per attendee. No refunds will be given for cancellations received after December 14, 2009. Participant substitutions may be made at any time at no additional charge. Substitutions and cancellations must be made in writing by email to register@cmsaglobal.org. Please Note: CMSA reserves the right to disclose and publish the names of those attending the Conference on CMSA's website, which is accessible to attendees only, and on an attendee list that will be distributed at the Conference.
